MCC Identity Document Certification

MCC Identity Document Certification: read the recipient’s request, bring the complete original, confirm the appointment.
Prepare your information, review the recipient’s instructions and keep a copy of your completed document.

Prepare for MCC’s identity confirmation process

If you are using physiciansapply.ca, read the identity-document request in your account before arranging certification. MCC’s identity guidance calls for a certified copy of primary identification, a photograph and a signature. Its certifying official must see you and your identity documents in person. Read MCC’s current identification requirements.

Bring the current instructions

Download any forms and certification wording from your own account. Do not rely on an old checklist or another applicant’s completed form. Tell our office which identity document you will present and whether MCC has asked you to update a previously accepted record.

  • The original identification specified by MCC.
  • The complete form or document request from physiciansapply.ca.
  • The copy or photograph requested by that checklist.
  • Supporting records for a name difference, if the request requires them.

At your Notaryspace appointment

Arrange an in-person appointment and bring the originals for inspection. Leave all certifying-official sections blank. Review your own factual entries beforehand, including name spelling and identification details. If the office needs clarification about a document or required statement, resolve that before signing.

Certification and credential verification

Certifying an identification copy is separate from MCC’s source verification of educational or professional credentials. A notarized copy should not be assumed to complete every step in your account. Continue to follow the individual request for each credential or service.

Submit and monitor the result

Follow the upload or delivery instructions in your physiciansapply.ca account, keep a readable copy and check for further requests. MCC determines whether the submitted documents meet its requirements; certification does not decide exam eligibility or medical registration. Contact our office with the current checklist to confirm your appointment and fee.
